I hereby understand, agree and confirm with the following Terms and Conditions:

  1. The above-mentioned details are entered/selected by me and will be updated in my Account.

  2. I hereby authorise ICICI Bank to contact me on the above-mentioned mobile number for verification, call back or checks to confirm the veracity of any transaction, as deemed fit by ICICI Bank.

  3. I authorise ICICI Bank to send all correspondence in relation to the Account on the communication address as updated in this application and shall indemnify ICICI Bank for any loss/damage/liability caused due to updation of the communication address by me in this application and for sending correspondences at the communication address updated herein.

  4. I agree that basis this request, my address details mentioned in Aadhaar will be updated as my permanent address in the Bank records.

  5. I have understood the information requirements of the application form and hereby confirm that the information provided by me is true, correct and complete.

  6. I hereby authorise ICICI Bank to send SMS alerts and e-mails to the above-mentioned mobile number and e-mail ID provided by me.

  7. Basis the documents/details submitted for my Account conversion (Minor to Major); Re-KYC status shall also be updated.

  8. I have authorised NSDL e-Governance Infrastructure Ltd (NSDL e-gov) to use my Aadhaar/Virtual ID details (as applicable) for execution of this application form, through Aadhaar based electronic signature.

  9. In compliance with the Rule 9B of the Prevention of Money Laundering (Maintenance of Records) Rules, 2005, as amended through notification dated March 7, 2023, I/We agree to intimate ICICI Bank in case of any update in KYC documents/information as submitted at the time of establishment of business relationship/Account-based relationship within a period of 30 (thirty) days (from the date of update) to enable updation in Bank records. Any such update shall be intimated to ICICI Bank by Business Centre/TAB/Digital Channel.

  10. All the information provided in the Application Form have been entered by me or at my directions. I verify and confirm that the said information is true and correct. I have not withheld any information required for converting my Savings Account (Minor to Major). I acknowledge that ICICI Bank (“the Bank”) reserves the right to reject the application at its sole discretion.

  11. I confirm that I have reviewed the transactions done in my Savings Account when I was a minor and I hereby ratify them.

  12. If my Savings Account is dormant, please activate the Savings Account basis this Application and the supporting documents.

  13. If for any reason, the Savings Account is overdrawn/has debit balance, I agree to repay such amounts along with applicable interest and charges. I also undertake to make repayments for any credit facilities availed by me, either via internet banking or otherwise and disbursed into my Savings Account maintained with the Bank in accordance with the applicable terms and conditions.

  14. I hereby authorise ICICI Bank to update my details for the existing Bank Account/s and any other facilities/product(s) availed from ICICI Bank basis the KYC document(s) and information provided with this Form. I restate my agreement to the terms and conditions governing each of such relationships/products/services.

  15. I am aware that the Savings Account/CAF shall also be governed by the 'Terms and Conditions for Savings Account' ("Terms for Savings Account"), tariff guidelines/schedule of charges and other applicable terms and conditions as available on the website www.icicibank.com ("Website"). I am further aware that Terms for Savings Account, tariff guidelines/schedule of charges may be modified and updated by ICICI Bank from time to time. I have read, understood and agree to abide by Terms for Savings Account, the tariff guidelines/schedule of charges, including the changes made to the same from time to time which will be updated on the Website.

  16. I hereby authorise the Bank to enable provisions of Internet Banking, Phone Banking and Mobile Banking services on the terms and conditions of Internet Banking services, Phone Banking and Mobile Banking as available/set forth on the Website and I agree to abide by the same. I understand that to revoke the usage of the above-mentioned services I shall have to issue a letter of revocation ("the revocation letter") to ICICI Bank in this regard. I hereby agree that until ten days after receipt of such revocation letter, the aforesaid authorisation shall hold good.

  17. I hereby declare that I authorise ICICI Bank to download/fetch my CKYCRR record for the purpose of processing of re-KYC/demog update application and/or updating the CKYCRR record reference number in Bank’s records or any other related purposes”

Aadhaar Consent:

I have been provided various options by ICICI Bank for establishing my identity/address proof for the purpose of Conversion of my Account (Minor to Major) with ICICI Bank and have voluntarily submitted my Aadhaar to ICICI Bank for biometric based e-KYC authentication and hereby give my consent to ICICI Bank:

  1. to establish my identity/address proof by Aadhaar based authentication system or verify the genuineness of the Aadhaar through Quick Response (QR) code or in such other manner as set out by UIDAI or any other law from time to time.
  2. share my Aadhaar details with UIDAI, NPCI, concerned regulatory or statutory authorities as may be required under the applicable laws I have been explained and informed in the local language understood by me about the consent and the purpose of collecting Aadhaar.

I have been informed that:

  1. Upon authentication, UIDAI may share with ICICI Bank, information in the nature of my demographic information including photograph which ICICI Bank may use as an identity/address proof for the above-mentioned purpose.

  2. My Aadhaar details (including my demographic information) shared by UIDAI will not be used for any purpose other than the purpose mentioned above or as per the requirement of law.

  3. My biometric information will not be stored by the Bank.
