How to Apply for Supplementary Credit Card

Step 1: Please click here to download the application form.

Step 2: You will have to submit the following documents along with the application form at the nearest ICICI Bank branch:

Photocopy of the Supplementary Card applicant’s Permanent Account Number (PAN) card,self-attested by the Supplementary Card applicant.
Photocopy of the Supplementary Card applicant’s Address Proof , self-attested by the Supplementary Card applicant.
Photocopy of the Supplementary Card applicant’s Permanent Account Number (PAN) card,self-attested by the Supplementary Card applicant is mandatory.

Keep the original copy of documents handy for verification.
